ATHLETICS

Requirements for Participation
Before any participation may occur, all athletic forms and certificates (listed below) must be in the Athletic office. These forms and certificates are:
- EL2 Physical
- EL3 Consent
- Medical Consent
- Insurance Card
- GA4 (Transfer)
Student-Athletes must watch each video and submit the certificate:
