Please note that although paying online with a debit or credit card is a convenient way to pay your re-enrollment fee, you will incur a 3% transaction fee in addition to the re-enrollment fee per student. You must submit this form for each student that is being re-enrolled. If you do not want to pay the transaction fee please submit your funds by check or wire transfer.
